- အသိသက်သေများ၏ နိုင်ငံကူးလက်မှတ်မိတ္တူနှင့်နိုင်ငံသားစိစစ်ရေးကတ်ပြားမိတ္တူ၊
- ထောက်ခံစာကြေးအမေရိကန်ဒေါ်လာ (၁၅)ဒေါ်လာ
လျှောက်ထားသူအနေဖြင့် သံရုံးသို့ လူကိုယ်တိုင်လာရောက်လျှောက်ထားရန် လိုအပ်ပြီးကောင်စစ် အရာရှိရှေ့မှောက်တွင် လူကိုယ်တိုင်လက်မှတ်ရေးထိုးရမည်ဖြစ်ပါသည်။

Requirements
(a) Request letter addressed to the Ambassador, Embassy of the Republic of the Union of Myanmar, Phnom Penh
(b) Curriculum Vitae of Applicant
(c) Original and copies of Passport (photo page and visa validity page)
(d) Revenue stamped document of General Power/ Special Power
(e) Signatures of 2 witnesses
(f) Particulars of applicants and witnesses such as Passport No, NRC No/ ID No, address and mobile number must be stated
(g) USD 15 for Attestation for Powers of Attorney
(h) Person authorizing General Power/Special Power must come to the Embassy in person and sign before the consular officer

- ထောက်ခံစာပြုလုပ်ရန် လိုအပ်သည့် စာရွက်စာတမ်းမူရင်း (နိုတြီထောက်ခံစာပြုလုပ်မည့် စာရွက်စာတမ်းများကို မြန်မာနိုင်ငံရှိနိုတြီပဗ္ဗလစ်၌ ရှေးဦးစွာနိုတြီပြုလုပ်ပြီးမြန်မာနိုင်ငံ၊ နိုင်ငံခြားရေးဝန်ကြီးဌာန၌ နိုတြီတုံးထုပြီးမှသာသံရုံးမှ ထပ်ဆင့်နိုတြီထောက်ခံချက်ပြုလုပ် ပေးမည် ဖြစ်ပါသည်။)
- နိုတြီထောက်ခံစာအတွက် ကောက်ခံငွေအမေရိကန်ဒေါ်လာ (၁၅)ဒေါ်လာ

(c) Original and copies of Passport (photo page and visa validity page)
(d) Original documents such as Birth Certificate/Marriage Certificate/Degrees/ Diploma, etc.) (Note, The documents need for attestation/ certification should be notarized by a registered Notary Public in Myanmar and endorsed by the Ministry of Foreign Affairs in Myanmar, prior to submission to the Myanmar Embassy.)
(e) USD 15 for Notary
