Loss/Damage of Passport

Myanmar Nationals whose passports are lost or damaged, shall apply new passports in person at the Myanmar Embassy. This case needs to be referred to the Passport Issuing Board in Myanmar to get approval for issuing new passport. The following documents are required with the duly-filled application form:

(a) Application Form (New Passport Apply Form)
(b) Curriculum Vitae
(c) three photos (white background and 4cmx6cm)
(d) Applicant’s letter stating detail of loss/damage
(e) Original Police report (for lost only)
(f) Copies of passport (Photo page and visa validity page)
(g) National Registration Card
(h) Copy of Households List
(i) Foreign Employment Card and Recommendation Letter from Work
(j) Recommendation Letter if the applicant is dependent
(k) (105) US Dollar should be submitted as the fine for the loss/damaged passport and (40) US Dollar for the new passport
