New Born Baby Passport

The parents of new born baby shall submit the application at the Embassy in person and this case needs to be referred to the Passport Issuing Board in Myanmar for issuing passport. The following documents are required with the duly-filled application form:

(a) Application Form (New Passport Apply Form)
(b) Curriculum Vitae (both parents and new born baby)
(c) Birth Certificate (Original and Copy)
(d) Marriage Certificate of parents (Original and Copy)
(e) National Registration Cards of Parents (Original and Copy)
(f) Copy of Household List
(g) Copies of parents’ passports (Photo page and visa validity page)
(h) Request Letter from parents
(i) Three photos each for parents and baby (white background and 4cmx6cm)
(j) USD 40 for new passport fee
